Our Intake Process

Step 1: 

Call Balance FMS or fill out an Intake Form.

Step 2:

Meet with Balance FMS.

Step 3: 

If SDP Participant and Balance FMS determine a good fit, then participant fills out onboarding packet.

Step 4: 

Completed documents are submitted to the Regional Center for approval.

Final Step: 

SDP Participant is now a Balance FMS client.
